HIPAA Notice of Privacy Practices
Gifted Skills Health Services, LLC provides mobile phlebotomy and health services in Virginia. This notice describes how we may use and disclose your protected health information and your rights regarding that information.
1. Our Commitment to Your Privacy
We are required by the Health Insurance Portability and Accountability Act ("HIPAA") and applicable state law to maintain the privacy of your protected health information ("PHI") and to provide you with this notice of our legal duties and privacy practices. We will not use or disclose your PHI except as described in this notice or as otherwise permitted by law.
2. How We May Use and Disclose Your PHI
We may use and disclose your PHI for treatment, payment, and healthcare operations. For example: we may share information with laboratories and ordering providers for treatment; with payers for payment; and internally for quality improvement, training, and business management. We may also disclose PHI as required by law, for public health activities, to avert a serious threat to health or safety, and for other purposes permitted or required by HIPAA and Virginia law.
3. Uses and Disclosures Requiring Your Authorization
Other uses and disclosures of your PHI will be made only with your written authorization. You may revoke an authorization in writing at any time, except to the extent we have already relied on it.
4. Your Rights Regarding Your PHI
You have the right to: (a) request access to inspect and obtain a copy of your PHI; (b) request an amendment of your PHI if you believe it is incorrect or incomplete; (c) receive an accounting of certain disclosures of your PHI; (d) request restrictions on certain uses and disclosures; (e) request confidential communications (e.g., by alternative means or location); and (f) obtain a paper copy of this notice upon request. We may require a written request for certain of these rights. We will not retaliate against you for exercising your rights.
5. Breach Notification
We will notify you and, where required, the Secretary of Health and Human Services, in the event of a breach of your unsecured PHI, in accordance with HIPAA and applicable state law.
6. Changes to This Notice
We reserve the right to change the terms of this notice. We will post the revised notice on our website and make it available at our offices. The revised notice will apply to all PHI we maintain.

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services.
